Your First Visit

It is our goal to help all our patients feel welcomed, safe, and treated well. Below is a step-by-step guide for scheduling your first appointment.

what will happen before your first visit?

First Phone Call: Please call our office at 518-489-5458 to make an appointment for your initial examination.

New Patient Forms: We will mail you a new patient package after the First Phone Call. This package includes New Patient Forms. To reduce your waiting time during your First Visit, please complete the New Patient Forms and bring them with you to your First Visit. For the New Patient Forms, please click here.

Confirmation Phone Call: We will give you a call for appointment confirmation a week prior to First Visit. We will (1) ask screening questions related to COVID-19, and (2) remind you to bring your dental insurance information. This information is very important for you to receive any reimbursement for the appointment from your insurance carrier. We are a fee-for-service practice and cannot guarantee coverage for any services. We encourage patients to contact their insurance companies to check coverage for out-of-network providers. Please Note: All patients under 18 must be accompanied by a parent or guardian at the consultation appointment.

What will happen on my first visit?

Location: We are conveniently located in the heart of Albany, accessible from various major highways. Our office address is 1295 Central Avenue, Albany, NY 12205 (at the intersection of Central Avenue and Ahl Avenue). Upon arrival, please use any parking space under the solar panel roof. When you enter the building, please check in with our administrative team. For COVID-19 precautions, we are asking all our patients to wear a mask upon entry. 

Please assist us at the time of your initial visit to the office by providing the following information:

  • Completed New Patient Forms
  • Your referral slip and x-rays from your referring dentist.
  • A list of medications you are currently taking (please note that you can fill out your patient registration and medical history form on our website.)
  • If you have dental insurance, please bring any forms or insurance cards with you to the appointment. It is very important that you have an accurate and updated dental insurance information for to receive any reimbursement from your insurance carrier.

Your First Appointment: Dr. Choe will perform a full periodontal examination.  He will review your dental and medical history, and examine your teeth, gums, and any x-rays we have received. Dr. Choe will explain any issues and help you make the best treatment decision.  If there is insufficient data to make a proper diagnosis during the First Visit, Dr. Choe may recommend updated radiograph(s) and/or another appointment for further examination.
